Are you a highly skilled and patient-focused Clinical Pharmacist seeking a dynamic role in healthcare? We are looking for an experienced Clinical Pharmacist to join our medical team and contribute to patient-centered medication therapy management. As a Clinical Pharmacist, you will play a pivotal role in optimizing therapeutic outcomes through direct interaction with physicians, nurses, and patients.

Key Responsibilities:

• Collaborate with healthcare teams to evaluate, develop, and manage individualized medication therapy plans.

• Review and interpret prescriptions, ensuring accuracy and appropriate dosing based on clinical guidelines.

• Provide evidence-based recommendations for medication selection, dosage, and monitoring.

• Monitor patients for adverse drug reactions, interactions, and therapeutic effectiveness.

• Educate patients about medication usage, side effects, and adherence strategies.

• Participate in hospital rounds, case discussions, and interdisciplinary team meetings.

• Document all clinical interventions and therapeutic decisions in patient records.

• Stay up to date with the latest developments in pharmacotherapy and clinical research.

Requirements:

• Doctor of Pharmacy (PharmD) degree from an accredited program.

• Active and unrestricted pharmacist license.

• Completion of a residency program (PGY1 required, PGY2 preferred).

• Experience in a hospital or clinical setting preferred.

• Excellent communication, decision-making, and problem-solving skills.

• Strong understanding of pharmacokinetics, pharmacodynamics, and evidence-based guidelines.
