We are seeking an experienced nurse practitioner to provide house calls, delivering personalized primary care to patients in a home-based setting. As a nurse practitioner for HouseCalls, you will offer high-quality care while building strong patient relationships.

Key Responsibilities:

• In-Home Assessments: Conduct comprehensive physical exams and health assessments in patients' homes.

• Chronic Disease Management: Monitor and manage chronic conditions such as hypertension, diabetes, and COPD.

• Medication Review: Perform medication reconciliation and evaluate treatment compliance.

• Preventive Care: Educate patients and families on wellness, lifestyle changes, and preventive health measures.

• Care Coordination: Collaborate with primary care providers, specialists, and caregivers to ensure continuity of care.

• Documentation: Maintain accurate electronic medical records in accordance with regulatory standards.

Requirements:

• Current and valid nurse practitioner license.

• Board certification as an NP (FNP, AGNP, or equivalent).

• A minimum of 1 year of experience in primary care or geriatrics is preferred.

• Strong interpersonal and communication skills.

• Ability to travel locally and conduct in-home visits.
